ABSTRACT

Background:

慢性心力衰竭(CHF)显著损害身体功能和生活质量。尽管基于运动的心脏康复是主要治疗策略,但由于后勤障碍,参与率依然偏低。数字健康技术(DHTs)为提供居家干预提供了有前景的替代方案。然而,关于它们对功能能力与日常身体行为具体影响的证据仍然不一致。

Objective:

本系统综述和荟萃分析旨在评估DHTs对充血性心衰竭患者身体活动和健康结果的疗效,并识别推动生理改善的干预特征。

Methods:

我们检索了Embase、CENTRAL、PubMed、Web of Science和JBI EBP数据库,查找2015年8月至2025年7月间发布的随机对照试验(RCT)。纳入了将DHT干预与常规护理或非数字主动对照组比较成年充血性心衰竭患者的研究。主要结局为功能性运动能力(6分钟步行距离[6MWD])、心肺健康(峰值氧气摄取[峰值VO₂])和每日步数。次要结局包括健康相关生活质量(明尼苏达心力衰竭生活问卷[MLHFQ]和堪萨斯城心肌病问卷[KCCQ])。数据通过随机效应模型综合,证据确定性采用GRADE方法评估。

Results:

共纳入20项RCT,共5293名参与者。DHT干预显著改善6MWD(平均差[MD] 17.61 m,95%置信区间9.84-25.39;P<.001)和峰值VO₂(MD 0.85 mL/kg/min,95% CI 0.70-1.00;与对照组比较P<.001)。亚组分析显示,综合混合远程康复模型在生理学上显著益处,异质性可忽略不计(I²=0%),而独立应用干预则表现出高度异质性和无显著趋势。DHT还显著降低了症状相关的生活质量负担(MLHFQ:MD –5.63;但对每日步数(P=0.37)和整体健康状况(KCCQ:P=.95)的影响不一致。<不良事件未报告显著增加。

Conclusions:

数字健康技术是改善充血性心衰竭患者心肺健康和功能能力的有效策略。然而,生理上的提升并不自动保证每天的体育活动量增加。这些干预措施的有效性似乎取决于具体的实施方式,采用远程监督的结构化混合模式优于基于应用的被动方法。未来的实施应优先考虑个性化、医疗监督的闭环管理系统。 Clinical Trial: CRD420251252258
