Designing A Mobile Health Application for Stroke Rehabilitation in Indonesia: A Mixed Methods Design Science Research Study
ABSTRACT
Background:
Indonesia ranks second in terms of stroke prevalence, with 4,918,487 people (1.7% of the population) living with stroke by 2025. This is in line with the condition in Indonesia, where there are limitations of health workers and health facilities in the stroke rehabilitation process that can be overcome through m-health.
Objective:
This study aims to design mobile health to help the stroke rehabilitation process in Indonesia.
Methods:
This study used a mixed-method Design Science Research methodology involving ten experts (doctors, therapists, health students) and eight post-stroke patients in an interview to explore the need for stroke rehabilitation management applications. The first iteration produced 13 features for patients and 12 features for medical personnel described in a low-fidelity prototype based on Intervention Theory with evaluation results obtained through review interviews with 16 medical personnel. The second iteration produced an application design design in a high-fidelity prototype based on the results of the first iteration review and obtained a System Usability Scale score of 71.83 ("GOOD" category) from 146 respondents supported by review interviews with 25 interviewees. The third iteration resulted in an improved application design based on the results of the evaluation in the second iteration and obtained an average score of 2.32 (system usefulness), 2.39 (information quality), 2.42 (interface quality), 2.35 (overall) which are "excellent" category from 128 respondents.
Results:
This study found features that can help patients rehabilitate, a training progress monitoring feature that can help patients find out their functional condition, speech and occupation, and a consultation service or home visit feature that can help patients rehabilitate with medical personnel. This study also developed other features such as authentication features, user profile management, schedule reminders, activity targets, educational texts, educational videos, therapy results reports, gamification, and onboarding.
Conclusions:
This study could provide guidance for health facilities and regulators in improving the quality of rehabilitation services that can be provided to stroke patients.
