Holistic and economic benefits of comprehensive obesity care: 6 month real-world outcomes from a digital UK Tier-3 service
ABSTRACT
Background:
Obesity is a complex chronic disease that disproportionately affects people living in socioeconomic deprivation. Although randomised trials demonstrate the efficacy of pharmacological and digital interventions, evidence on their real-world effectiveness regarding behavioral mechanisms, holistic health outcomes, and economic impact in routine care settings remains limited.
Objective:
This real-world evidence study evaluated 738 adults enrolled in the Oviva UK Tier-3 digital service.
Methods:
Outcomes at baseline, 3 and 6 months included body weight, lifestyle behaviors, psychosocial health, sickness-related absence from work, healthcare use, and engagement with Oviva App. Correlational and multivariable models examined predictors of weight change.
Results:
Participants achieved clinically meaningful weight loss (-2.7% at 3 months; -8.6% at 6 months). Improvements were also observed across general and mental health, physical functioning, quality of life, dietary habits, alcohol intake, sickness-related absence, and healthcare utilisation (all P<.05). Sick days decreased by 50% after six months of Oviva care. Greater self-monitoring and Oviva App engagement predicted larger weight losses.
Conclusions:
The digital Tier-3 service produced substantial weight loss and broad behavioral, psychosocial and economic benefits. Engagement with Oviva App was a key driver of success, supporting the role of scalable Tier-3 services in expanding access and enhancing NHS obesity care.
