ABSTRACT
Background:
The state of the stool during bowel preparation (BP) should be properly evaluated to perform colonoscopy with sufficient quality. Therefore, we developed a smartphone app to evaluate a patient’s stool state during BP and a viewer to enable real-time monitoring by medical staff.
Objective:
The objective of this study was to assess the feasibility of performing colonoscopy of appropriate quality using the app and viewer system.
Methods:
This prospective observational study was conducted between November 2022 and December 2023, involving patients scheduled for colonoscopy at 10 facilities in Japan. The patients downloaded the app on their smartphones and captured their stools during BP, while the medical staff reviewed the evaluation of the stools by the app via the viewer system. The primary endpoint was the proportion of patients with a Boston Bowel Preparation Scale ≥ 6. Additionally, we evaluated the usability of the app, the medical staff workload burden with the app, and viewer usage via questionnaire surveys.
Results:
In total, 326 patients were included in the analysis, of which 99.1% (95%CI: 97.3-99.8%) achieved the primary endpoint. In the questionnaire survey, 295 patients (90.5%) indicated that they wanted to use the app for their next colonoscopy. Moreover, 60 of 67 (89.6%) medical staff who answered the questionnaire reported a reduction in workload burden.
Conclusions:
The app and viewer are expected to help ensure colonoscopy of sufficient quality and allow real-time monitoring of BP progress, leading to a reduction in the burden on patients and medical staff.
