Using Community-Based Participatory Research to Co-Develop Hypotheses and Patient-Centered Outcomes Addressing Implementation Gaps in the Prevention of Acute Kidney Injury

ABSTRACT

Background:

Acute kidney injury (AKI) is a common renal impairment associated with diagnostic and interventional catheterization procedures. About 14% of all patients undergoing such procedures and about 50% of patients with pre-existing chronic kidney disease could develop AKI. Evidence-based interventions (EBIs) to prevent acute kidney injury are well-researched, considered standard of care, and endorsed by US and international nephrology and cardiovascular professional organizations. Despite established EBIs, they are inconsistently applied in clinical practice.

Objective:

This study utilized community-based participatory research to identify research questions, hypotheses, and patient-centered outcomes to improve uptake and maintenance of AKI EBIs in real-world clinical settings.

Methods:

In total, four patients and eight clinicians/scientists attended a one-hour virtual event to rank implementation models on best practices, real-world needs of patients, accessibility, and effectiveness.

Results:

Two implementation models emerged that met all criteria: Collaborative with Surveillance vs. Technical Assistance (standard of care).

Conclusions:

Choosing between these models presented a decisional dilemma, and this study provides the groundwork for a comparative effectiveness study.
