The Next Frontier of Remote Patient Monitoring: Hospital at Home

ABSTRACT

Remote patient monitoring (RPM) has shown promise in aiding safe and efficient remote care for chronic conditions, however its use remains largely unexplored within Hospital at Home (HaH) programs despite a significant opportunity to increase patient eligibility for and decrease the cost of HaH. This article describes how HaH could further adopt the three primary modalities of RPM (vital sign, telemetry and fall monitoring) to safely expand appropriate patient enrollment in HaH programs. We also detail how by using RPM, HaH may deliver care more efficiently and achieve cost-savings. For RPM to achieve its potential in enhancing HaH care delivery, further care delivery research which could inform optimization of patient – RPM modality matching and implementation considerations as well as enabling policy considerations should be considered.
