A Cost-Effectiveness of Mobile Phone Based Digital HIV Prevention Among Men Who Have Sex With Men in China
ABSTRACT
Background:
The cost-effectiveness of HIV risk prediction-based e-health interventions among men who have sex with men (MSM) has not been examined for a long time frame.
Objective:
We evaluated the cost-effectiveness of an e-health intervention strategy for MSM in China.
Methods:
We used a model with China-specific data to project the clinical impact and cost-effectiveness of two strategies targeting MSM, including a comprehensive intervention based on HIV risk prediction and prevention knowledge education. Outcomes of interest were the number of HIV infections averted by e-health intervention, intervention implementation costs and costs-per-HIV infection averted by e-health intervention. Univariate and multivariate sensitivity analyses were also conducted to evaluate the influencing factors.
Results:
On the basis of the mathematical model, it is estimated that the cost of intervention for 10,000 MSM for 1 year is 798,960 yuan and the per-person cost of the intervention is 79.9 yuan. The intervention can prevent HIV infection in 48 MSM, which could save 7,923,216 yuan and 480 quality-adjusted life years (QALYs). The cost of avoiding 1 case of HIV infection is 16,645 yuan, and the cost-utility ratio is less than 0. Sensitivity analysis showed that the cost-effectiveness of risk prediction-based e-health interventions is mostly impacted by the average number of sexual behaviors with each sexual partner. Additionally, the higher the HIV prevalence among MSM, the greater the benefit of the intervention.
Conclusions:
Risk prediction-based e-health intervention is a cost-effective HIV prevention strategy for MSM and could be considered for promotion and application among high-risk MSM subgroups. Clinical Trial: ChiCTR1800017268
