Development of a Malaysian Healthcare Quality Performance Framework for a Composite Index: A Study Protocol
ABSTRACT
Background:
Measuring healthcare quality is inherently complex due to the interdependence of multiple factors. Nevertheless, systematic, reliable, standardized, and context-specific measurements are essential for assessing and improving healthcare quality.
Objective:
This study aims to develop a framework for a composite index to comprehensively assess healthcare quality in Malaysia across the 7 local quality domains: Safe, Timely, Effective, Efficient, Equitable, People-centered, and Accessible (STEEEPA).
Methods:
This study will be conducted in four stages covering (1) the indicator compilation stage and assignment of preliminary STEEEPA domains, (2) indicator prioritization and selection stage using the modified Delphi method, (3) assessment of the prioritized indicator set, and (4) assigning weightages by applying the Analytic Network Process (ANP) using indicators across 7 local quality STEEEPA domains. An expert panel will be involved in the modified Delphi process and will subsequently conduct pairwise comparisons of quality indicators mapped to the STEEEPA domains to derive priority weights for each domain and indicator. Consistency ratios will be calculated to assess the reliability of expert judgments. A weighted supermatrix will be computed and normalized to derive the final weights for the indicators.
Results:
The study is currently in the initial stage of indicator compilation. It will generate a compilation of indicators, each assigned to its respective STEEEPA domain, serving as the basis for prioritization. The ANP analysis will then commence and is expected to generate weightages for each indicator within its respective quality domain. At the end of the study, a framework for generating a healthcare quality performance composite index will be developed, comprising the STEEEPA domains and their associated indicators.
Conclusions:
The developed framework will enable the generation of a Malaysian healthcare quality performance composite index, allowing comparison of performance across the STEEEPA domains over time and at different levels of the healthcare system. It will also allow the identification of areas requiring further improvement and support targeted interventions to improve healthcare quality.
