Association between Fatty Liver Index and Incidence of Cataract Surgery in Individuals Aged 50 Years and Older based on the Korean NHIS-HEALS Data: Longitudinal retrospective Cohort Study

ABSTRACT

Background:

Cataract is a leading cause of vision impairment. Obesity-related risk factors, including insulin resistance, increase the risk of cataract. The study aimed to investigate the association between Fatty Liver Index (FLI) as an indicator of fatty liver disease and the occurrence of cataract surgery.

Objective:

This retrospective cohort study for the association between FLI and cataract surgery provides that a higher fatty liver index is associated with an increased incidence of cataract surgery in adults aged 50 years or older.

Methods:

This study was retrospectively designed based on the Korean NHIS-HEALS cohort (median follow-up 9.8 years). Participants were assigned to one of three groups based on FLI: low (<30), intermediate (30-<60), or high (≥60). Kaplan-Meier survival analysis was performed on the cumulative incidences and multivariable Cox proportional hazards regression models were used to study the association between FLI group and cataract surgery after adjusting for potential confounders.

Results:

Of the 139,079 included participants, cataract surgery incidence was 12.49%, 13.95%, and 14.16% in the low, intermediate, and high FLI groups, respectively. Hazard ratios (HRs) (95% CIs) of intermediate and high FLI groups for all-cataract surgery were 1.052 (0.984–1.124) and 1.111 (1.029–1.200) in men, respectively, and 1.050 (0.995–1.107) and 1.168 (1.086–1.256) in women after adjustment for all confounding factors. HRs (95% CIs) of intermediate and high FLI groups for senile-cataract surgery were 1.032 (0.964–1.105) and 1.105 (1.022–1.196) in men, respectively, and 1.000 (0.946–1.056) and 1.139 (1.057–1.227), in women after adjustment for all confounding factors.

Conclusions:

Individuals with a higher FLI had a higher risk of all-cataract surgery. This association was maintained even after limiting the analyses to senile-cataract surgery. Clinical Trial: The Institutional Review Board of Chungbuk National University Hospital approved the present study (CBNUH 2023-08-035). We do not register at ClinicalTrials.gov.
