Rapid implementation of an innovative virtual diabetes “Boot Camp” program
ABSTRACT
Background:
COVID-19 disrupted healthcare, causing a decline in the health of chronic disease patients and a need to reimagine diabetes care. In
Objective:
In less than three months, from project conception to the enrollment of our first patients, we created a virtual boot camp to improve diabetes care. Our aim is to provide strategies for launching a rapid virtual program that can be adopted quickly by healthcare organizations.
Methods:
A multidisciplinary team of physicians, dieticians, and educators, with support from telehealth team created a virtual program for diabetic patients. The program combined online diabetes data tracking with weekly telehealth visits.
Results:
Over a hundred patients have been enrolled virtual diabetes boot camp. Preliminary data show improvement of diabetes in 75 % who completed the program. Four principles were identified and developed to reflect the quick design and launch.
Conclusions:
The rapid launch of a virtual diabetes program is feasible. We propose practical strategies and guidelines using a coordinated, team-based, systematic approach for quick implementation and sustained adoption across a large multispecialty ambulatory healthcare organization.
