ABSTRACT

Background:

Resveratrol is a natural polyphenol mainly found in grapes, red wine, and berries. Experimental studies suggest that resveratrol modulates key pathways related to oxidative stress, inflammation, insulin signaling, and endothelial function. However, despite promising findings, evidence from human studies remains inconsistent, and substantial methodological heterogeneity persists across randomized controlled trials and previous evidence syntheses.

Objective:

This systematic review aims to provide an updated and integrated synthesis of randomized controlled trials evaluating the effects of oral resveratrol supplementation on inflammatory biomarkers and cardiometabolic outcomes in adults, while exploring potential methodological and biological sources of heterogeneity across studies

Methods:

This protocol is reported in accordance with the preferred reporting items for systematic review and meta-analysis protocols guidelines. Randomized controlled trials evaluating oral resveratrol supplementation compared with placebo, other nutritional supplements, or no intervention in adults will be included. Searching will be conducted in PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), Scopus, Web of Science, and SciELO. Risk of bias will be assessed using the Cochrane Risk of Bias 2 tool (RoB 2). The certainty of evidence will be evaluated using the Grading of Recommendations Assessment, Development and Evaluation approach (GRADE).

Results:

Literature searches and study selection are currently ongoing. Completion of the systematic review is expected in late 2026

Conclusions:

By addressing methodological variability and potential sources of inconsistent findings across randomized controlled trials, this review may improve the interpretation and clinical applicability of current evidence regarding oral resveratrol supplementation in adults Clinical Trial: This systematic review protocol has been registered in the International Prospective Register of Systematic Reviews (PROSPERO), registration number CRD420251178811
