Bridging Care and Cost: Evaluating the Clinical and Financial Benefits of Remote Monitoring and Chronic Care Strategies

ABSTRACT

Chronic disease poses a significant global health challenge, with remote patient monitoring (RPM) offering a promising solution for chronic care management (CCM). By leveraging wearable devices and sensor-based systems to continuously track vital metrics, RPM, combined with artificial intelligence and predictive analytics, enables real-time interventions and preventative care. This narrative review examines the physiological outcomes and cost savings associated with RPM by synthesizing findings from recent cohort studies and other published literature. Key findings reveal improvements in patient health outcomes, reduced hospital admissions, and cost savings by decreasing emergency interventions and optimizing care delivery, highlighting RPM’s potential to enhance both clinical efficacy and healthcare efficiency.
