Community-Driven HPV Vaccination: A Living Systematic Review Protocol Using Ward Health Approaches

ABSTRACT

Background:

Human papillomavirus (HPV) vaccination is critical in the prevention of cervical cancer and other HPV-related diseases. Despite the availability of the HPV vaccine, significant barriers—such as vaccine hesitancy and limited community involvement—impede widespread vaccination uptake in Nigeria. These challenges underscore the need for strategies that effectively engage communities and address vaccine hesitancy.

Objective:

This living systematic review protocol aims to evaluate the effectiveness of utilizing the Ward Health System (WHS) approach to enhance community engagement and increase HPV vaccination acceptance in Nigeria.

Methods:

The review will adopt a comprehensive, iterative approach, starting with an initial systematic review and followed by bi-monthly updates over a 12-month period. Searches will be conducted across six major databases, including PubMed, Embase, Scopus, CINAHL, Web of Science, and African Journals Online, along with additional grey literature and preprint sources. Data will be synthesized through narrative synthesis, and meta-analysis will be performed where suitable. The quality of included studies will be assessed using the Mixed Methods Appraisal Tool (MMAT).

Results:

Findings from this living review will provide continuous, up-to-date evidence on the effectiveness of the WHS approach in enhancing HPV vaccination rates in Nigerian communities.

Conclusions:

This protocol outlines a systematic approach to evaluating the WHS model’s impact on HPV vaccine uptake, potentially informing community engagement practices to address vaccine hesitancy and improve vaccination coverage. Upon completion, the findings from this review are expected to provide evidence-based recommendations for healthcare policymakers and practitioners on using WHS to enhance HPV vaccination acceptance in Nigeria. This review addresses a crucial gap in HPV vaccination uptake by assessing a community-based engagement model within Nigeria’s healthcare system. The findings may have broader implications for HPV vaccination strategies across similar low-resource settings.
