Exploring Clinical Perspectives on Technology-enabled Home-Based Heart Failure Self-Care for Older Adults: Qualitative Study
ABSTRACT
Background:
Heart Failure (HF) is one of the leading causes of hospitalization and mortality in older adults. Digital Health tools such as mHealth applications can support patients with HF to manage their symptoms and health at home. A mHealth tool aimed and designed specifically for older patients with HF supports self-care in the home setting according to daily weight measurements and symptoms. Healthcare providers (HCPs) such as nurses and family physicians (FPs) are key clinical perspectives that need to be considered when implementing digital health tools as they are in the most direct contact with patients with HF.
Objective:
The purpose of this study was to obtain feedback from valuable HCPs such as nurses and FPs to incorporate into a mHealth tool that promotes self-management for patients with HF.
Methods:
This qualitative study was guided by an evidence-based user-centered design (UCD) framework. The study format followed a semi-structured interview discussion with nurses and FPs which were then transcribed verbatim using NVIVO12.
Results:
Based on our analysis, we identified the following themes: widening patient care challenges, reducing workload burden, customizing the self-care journey for patients and enabling data access. Widening patient care challenges emphasize the importance of creating a simple digital health tool that is easily incorporated into a patient’s routine. Reducing workload burden refers to the ability of a digital health tool to reduce the number of patient clinical visits as the tool would empower and educate patients, with clear guidelines and expectations in place. Customizing patients’ self-care journey is the acknowledgement of a diverse patient population which consists of unique preferences and varying levels of physical and cognitive capabilities which must be accounted for. Lastly, enabling data access is crucial for HCPs as it provides a comprehensive view of patient data, however, liability concerns were expressed regarding the monitoring of patient data.
Conclusions:
We identified key themes that are essential when developing and implementing digital health tools, specifically mHealth applications, for the older patient population by considering key clinical perspectives.
