Leveraging Federated Learning, Explainable Artificial Intelligence, and Governance Requirements for Privacy-Preserving Multisite Clinical Diagnostic Platforms: A Scoping Review Protocol
ABSTRACT
Background:
Background:
Multisite diagnostic artificial intelligence must address data protection, explanation, subgroup performance, interoperability, and clinical governance alongside predictive performance. Federated learning can enable collaborative training without centralizing raw patient records, but deployment evidence across these domains is heterogeneous.
Objective:
Objective:
To map technical, implementation, and governance evidence and derive prioritized requirements for privacy-preserving multisite clinical diagnostic platforms.
Methods:
Methods:
This scoping review will follow established guidance. MEDLINE, Embase, IEEE Xplore, the ACM Digital Library, Scopus, Web of Science Core Collection, and the Cochrane Library will be searched for English-language sources from January 1, 2017. Supplementary searching will cover preprints, technical documentation, standards, operative regulatory sources, and citation networks. Sources must address at least one relevant technical or governance component applied to, or intended for, health data. Two reviewers will independently screen and chart sources. Narrative synthesis will map architectures, privacy mechanisms, explainability, fairness, interoperability, validation, regulation, and implementation. A review-specific five-level maturity framework will characterize deployment evidence; no conventional risk-of-bias assessment or meta-analysis is planned.
Results:
Results:
The protocol was registered on September 18, 2026. Database searching was underway as of October 12, 2026. The registered anticipated completion date is March 31, 2027. No review findings are reported in this protocol.
Conclusions:
Conclusions:
The review will provide a traceable requirements framework that distinguishes empirical support, operative governance requirements, and reviewer inference, with explicit attention to settings with variable resources and connectivity.
